Abstract.

We report the case of a 46-year-old man who was admitted to the medical department of the hospital Reichenbach with a 2-day history of central and upper abdominal pain. The pain was displaced in the right lower abdominal quadrant, which is why the patient was transferred to our department. At laparoscopy a toothpick was found perforating the cecum. Laparoscopic management, therapeutic possibilities and problems of diagnosis are discussed. The patient does not recall having ingested a toothpick. The laparoscopic treatment of perforation of the cecum by a toothpick might be among the rarest operations ever conducted. In the literature no such publication could be found up to now.

Zusammenfassung.

Wir berichten über einen 46 jährigen Patienten mit chronischem Alkoholabusus, der wegen seit zwei Tagen bestehenden rechtsseitigen Mittel- und Oberbauchbeschwerden zunächst in der Inneren Abteilung des Kreiskrankenhauses Reichenbach stationär aufgenommen wurde. Nach Verlagerung der Schmerzen in den rechten Unterbauch erfolgte die Übernahme in die Chirurgische Abteilung. Unter der Diagnose einer akuten Appendicitis wurde der Patient laparoskopiert. Als Ursache der Beschwerden fand sich eine Perforation des Coecums durch einen Zahnstocher. Dieser wurde laparoskopisch geborgen, die Perforationsstelle übernäht und die mitbeteiligte Appendix entfernt. Die orale Aufnahme des Zahnstochers war dem Patienten nicht bewußt. Diagnostische Probleme und therapeutische Möglichkeiten werden diskutiert. Die durchgehend laparoskopische Versorgung der Perforation des Coecums durch einen Zahnstocher dürfte zu den seltensten chirurgischen Eingriffen zählen. In der uns bekannten Literatur ist eine entsprechende Veröffentlichung nicht zu belegen.
