Olanzapin in der Behandlung depressiver Störungen mit psychotischer Symptomatik

Zusammenfassung

Es wird über die erfolgreiche Behandlung von 2 Patientinnen mit psychotischer Depression mit dem neuen atypischen Neuroleptikum Olanzapin berichtet. Bei einer 75jährigen Patientin mit rezidivierender endogener Depression, die mit taktilen Halluzinationen und Zönästhesien einherging, zeigte sich während eines 39wöchigen stationären Aufenthaltes trotz einer systematischen mehrstufigen antidepressiven Behandlungsstrategie kein Therapieerfolg. Unter Zugabe von Olanzapin zu dem SSRI Citalopram (im Sinne einer Zweizügeltherapie) kam es zu einem raschen und anhaltenden Rückgang der refraktären depressiven Symptomatik sowie der taktilen Halluzinationen und Zönästhesien. Im zweiten Fall einer 57jährigen Patientin mit wahnhafter Depression kam es unter einer 4wöchigen Olanzapin-Monotherapie zu einer deutlichen Symptomreduktion und Korrektur des Verarmungswahns. Vor dem Hintergrund der vorgestellten Kasuistiken wird Olanzapin mit Hinblick auf sein Rezeptorbindungsprofil (u.a. 5-HT2A- und D1–D4 Rezeptor-Antagonismus) als geeignetes Neuroleptikum bei der Behandlung von affektiven Störungen mit psychotischer Symptomatik diskutiert.

Summary

We report on the successful treatment of two female patients with psychotic depression with the new atypical neuroleptic drug olanzapine. A 75-year-old female inpatient suffering from recurrent endogenous depression with tactile hallucinations and coenesthesia was refractory to a systematic sequential antidepressant treatment strategy during a 39-week period. After addition of the new atypical neuroleptic olanzapine to the SSRI citalopram, she showed immediate and ongoing symptom relief. In the second case of a 57-year-old female inpatient suffering from delusional depression, we observed marked symptom relief and correction of the delusions of impoverishment. Olanzapine is discussed with regard to its receptor-binding profile (antagonism to 5-HT2A- and D1–D4 receptors) as a potential neuroleptic drug in the treatment of affective disorders with psychotic symptoms.
