Implikationen für die psychotherapeutische Behandlung

Zusammenfassung

Der Myokardinfarkt ist ein plötzlich eintretendes lebensbedrohliches Ereignis (Mortalität = 50% innerhalb der ersten 28 Tage). Fast immer sind die akuten Infarktbeschwerden so stark und bedrohlich, dass den Betroffenen die Gefahr ihrer Situation wenigstens teilweise bewusst wird. Mit dem Eintreffen des Notarztwagens beginnen heute Diagnostik und Therapie. Die Fortschritte der interventionellen Kardiologie führen zu einer hoch effektiven Behandlung (Angiographie mit anschließender Dilatation/Bypassoperation und/oder medikamentöser Behandlung) und Rehabilitation. In den ersten 6 Wochen haben die Betroffenen bedrohliche und viele Lebensbereiche betreffende Informationen zu verarbeiten und müssen sich (oft erstmals im Leben) mit der Angst vor dem Tod auseinander setzen. Schon früh wurde in der wissenschaftlichen Literatur das Infarktereignis als ein psychisch schwer belastendes und bei einigen Menschen traumatisches Geschehen angesehen. Nachfolgend wird auf der Basis einer systematischen Literaturrecherche der Stand des Wissens zu der Frage einer posttraumatischen Belastungsstörung (PTSD) nach Infarkt referiert: Es muss damit gerechnet werden, dass ca. 11% der Betroffenen eine PTSD entwickeln. Es werden die psychotherapeutischen Implikationen diskutiert.

Abstract

An acute myocardial infarction is an unexpected life threatening event (mortality = 50% within the first 28 days). Frequently the acute symptoms could be so strong that the danger gets conscious to the persons affected. Diagnostics and therapy start with the arriving of the emergency doctor’s car today. The progress of interventional cardiology leads to hospital treatment (angiography with following dilatation/bypass crafting and/or medical treatment) and rehabilitation, which is very effective. Persons affected have to cope with serious informations (“information trauma”) and have to face death often for the first time in the life. An infarction as an emotionally threatening event has been discussed in the scientific literature from the beginning of our century. In the last years the question of posttraumatic stress disorder rises. The following paper gives a systematic review to this issue: About 11% of the patients develop a PostTraumatic Stress Disorder (PTSD). The psychotherapeutic implications are discussed.

  1. Eine sehr interessante autobiographische Darstellung des Umgangs mit der Todesangst findet sich bei Georg Engel, einem sehr bekannten Psychosomatiker, der das Konzept der Hilf- und Hoffnungslosigkeit („given-up, giving-up“) gemeinsam mit Schmale entwickelt hat, dessen Zwillingsbruder 40-jährig an einem Myokardinfarkt starb und der selbst ein Jahr später einen Infarkt überlebte und schließlich 86 Jahre alt wurde (Engel 1975).

  2. Es ist allerdings anzumerken, dass im klinischen Alltag dieser strengen Interpretation der ICD-Logik häufig nicht mehr gefolgt wird und die nächste Revision vermutlich eine Angleichung der Systeme bringen wird.

  3. Recherchierte Arbeiten die verwandte Konstrukte im Sinne einer Anpassungsstörung (Such-Term „emotional near1 distress“) behandeln, wurden nicht aufgenommen. Auch Arbeiten, die der Frage nachgehen, ob eine PTSD Auswirkungen auf das kardiovaskuläre System hat, wurden nicht aufgenommen. Bei den Recherchen wurden unter den genannten Such-Terms auch Arbeiten gefunden, die die psychische (traumatische) Verarbeitung bei Defibrillatoren, Herzstillstand, Herztransplantation oder Bypassoperationen thematisieren. Diese wurden nicht berücksichtigt. Weiterhin wurden Arbeiten ausgeschlossen, die die Folgen schwerer Belastungen (v. a. Kriegsteilnahme) auf die spätere Entwicklung einer koronaren Herzerkrankung untersuchten und dabei Personen mit und ohne vorherige PTSD verglichen.
