Abstract

The heritability of left ventricular structure and function was studied in 53 pairs of monozygotic and dizygotic twins, aged 18 to 31 years, by means of M-mode and Doppler echocardiography. A significant fraction of the phenotypic variances of left ventricular wall thickness was explained by genetic variance. For the left ventricular internal dimension, however, no genetic effect was found after correction for body weight. After appropriate adjustment a genetic component of 32% was found for aortic peak flow velocity, 55% for stroke distance, and 47% for minute distance. The early and late diastolic peak flow velocities at the mitral valve showed a genetic component estimated at 43% and 26%, repectively. The inheritance of the late/early diastolic peak flow velocity ratio was estimated at 53%. In conclusion, these data suggest a significant genetic component for left ventricular structural features but not for cavity dimension. A significant influence of heredity was detected for left ventricular inflow and outflow characteristics.

However, the factors that determine cardiac diastolic function have not been completely elucidated. Several studies have demonstrated heritability or intrafamilial aggregation of various aspects of left ventricular (LV) diastolic function identified from transmitral blood flow velocity measurements.12–18 However, contemporary evaluation of LV DD also includes E/velocity of myocardial tissue lengthening at the mitral annulus (Ea or eʹ) (E/eʹ), an index of filling pressures used to identify moderate-to-severe DD and which is independently associated with the development of heart failure with a preserved EF.19,20

This is in agreement with Jin et al.,2 who demonstrated moderate heritability for LV mass (multivariate-adjusted heritability of 23%). Bielen et al.5 showed a significant genetic contribution to myocardial wall thickness but no influence of heritability on LV internal diameters, for which environmental effects were shown. Overall, small to moderate heritability has been shown for LV structure.

Applying these cutoffs to the entire study population, this would include 5.7% of parents and 4.5% of siblings, all of whom were from case families. The heritability of the echocardiographic measurements of selected cardiac structures has been previously examined.35-44 For example, heritability has been estimated to be 0.96 for aortic root measurements, 0.72 for sinotubular junction measurements, and 0.57 for aortic valve annular measurements in first-degree relatives of children with left ventricular outflow tract obstructive malformations.44

One of the main directions was the functioning of the circulatory and respiratory systems. Research on male twins showed that, except for left ventricular internal diameter, all heart structures were significantly influenced by genetic factors, with heritability levels ranging from 29 to 68% [21]. Other cardiac structures and functions (i.e. heart rate, peak aortic flow velocity, stroke distance and minute distance (understood as the distance that can be run in one minute) also showed a significant genetic effect, which ranged from about 50 to 60% [3].
