Dimensions of suicidal ideation in psychiatric inpatients

Abstract

The computer-assisted version of the Beck Scale for Suicide Ideation (BSI; Beck, Steer & Ranieri, Journal of Clinical Psychology, 44, 499–505, 1988) was administered to 330 psychiatric inpatients diagnosed with mixed disorders. The BSI is a 19-item self-report instrument for measuring the intensity, duration and specificity of a patient's thoughts and plans about committing suicide. Of the 330 inpatients, 115 (34.8%) were classified as suicide ideators according to the BSI. Controlling for gender, a maximum-likelihood principal factor analysis of the partial intercorrelation matrix among the ideators' BSI items was conducted using an oblique rotation. Three factors were found that reflected Desire for Death, Preparation for Suicide and Active Suicidal Desire. The importance of assessing specific factors of suicidal ideation was discussed.

    Our analysis revealed a two-factor structure for the SSI-C and the SSI-W. Items 6, 7, 9, 10, 12, 14 and 18 had poor correlations according to the SSI-C, but in the SSI-W, they had good correlations with each factor. Our factor structure results are similar to those of Beck et al. [16], whereas Steer et al. [27] believed that the SSI should have a three-factor structure, and Marc et al. [28] thought that the SSI should contain only one factor. Different conclusions may be drawn based on different countries, different cultures and different populations.

    Passive suicidality deals with vague thoughts of suicide, such as thoughts of death and feelings that life is not worth living. Having ‘thoughts of death’ is itself part of the DSM-5 criteria for MDD (American Psychiatric Association, 2013) and such passive suicidality is associated both with active suicidal ideation and with suicidal acts themselves (Steer et al., 1993). A high-frequency intra-day measure of passive suicidal thoughts may therefore allow for a more nuanced measurement of suicidality in MDD.
