Abstract

The authors describe an 11-item short form of the Profile of Mood States' 58-item Total Mood Disturbance Score (TMDS). The Brief TMDS was derived from a sample of 619 adults with mixed cancer diagnoses, and replicated on a second sample of 295 lung cancer patients. Internal consistency of the Brief TMDS and the correlations of the Brief TMDS with the full TMDS were highly satisfactory for both samples. Given the difficulty many medically ill people have with lengthy self-report scales, and the increasing importance of measuring distress as an adjunct to patient care, this measure shows promise as a rapid, reliable tool.

Symptoms of anxiety and fear are a common source of suffering in patients with advanced, life-threatening illness. Careful assessment of anxiety as a potential source of distress is warranted. If left untreated, anxiety and fear may lead to poor care outcomes and prognoses, significant impairments in physical and psychosocial functioning, and a decreased quality of life. A multimodal treatment that utilizes psychotherapy and other nonpharmacological treatments, and when necessary, the addition of pharmacological treatments, can provide vital relief.
Comparing internal and external validation in the discovery of qualitative treatment-subgroup effects using two small clinical trials

In a two-arm randomized trial where both arms receive active treatment (i.e., treatments A and B), often the primary goal is to determine which of the treatments, on average, is more effective. A supplementary objective is to understand possible heterogeneity in the treatment effect by identifying multivariable subgroups of patients for whom A is more effective than B and, conversely, patients for whom B is more effective than A, known as a qualitative interaction. This is the objective of the qualitative interaction trees (QUINT) algorithm developed by Dusseldorp et al (Statistics in Medicine, 2014). We apply QUINT to a small randomized trial comparing facilitated relaxation meditation to facilitated life completion and preparation among patients with life-limiting illness (n = 135). We then conduct an internal validation of the QUINT solution using bootstrap resampling and compare it to an external validation with another, similarly conducted small randomized trial. Internal and external validation showed the apparent range in effect sizes was over-estimated, and subgroups identified were not consistent between the two trials. While the qualitative interaction trees algorithm is a promising area of data-driven multivariable subgroup discovery, our analyses illustrate the importance of validating the solution, particularly for trials with smaller numbers of participants.
Psychology of return to play after anterior cruciate ligament injury

The return to play process following an anterior cruciate ligament (ACL) injury can often take longer than expected because of psychological versus physical readiness. Research has shown mood disturbance, anxiety, fear of reinjury, locus of control, self-efficacy, self-confidence, motivation, and self-determination to be the potential factors influencing return to play following ACL injury. This chapter focuses on each of these areas with presentation of pertinent research findings, self-report measures that can be used for assessment, and considerations specifically for women recovering from ACL injuries. Identification of these underlying psychological difficulties that can impede the return to play process by the physician is the theme of this chapter, in order to facilitate appropriate referrals to sport psychologists and/or other resources who may assist athletes in returning to play sooner following ACL injuries. A brief description of what sport psychologists are able to offer is also included.
Informal caregiving in COPD: A systematic review of instruments and their measurement properties

Increasing symptoms and activity restriction associated with COPD progression greatly impact on the lives of their informal caregivers, who play a vital role in maintaining their health. An understanding of this impact is important for clinicians to support caregivers and maintain a viable patient environment at home. This systematic review aimed to identify the instruments commonly used to assess informal caregiving in COPD and describe their measurement properties in this population.
Searches were conducted in PubMed, Scopus, Web of Science, CINAHL and PsycINFO and in references of key articles, until November 2016 (PROSPERO: CRD42016041401). Instruments used to assess the impact of COPD on caregivers were identified and their properties described. Quality of studies was rated using the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) checklist. Quality of the measurement properties of instruments was rated as ‘positive’, ‘negative’ or ‘indeterminate’.
Patients cared for, had moderate to very severe COPD and the sample of caregivers ranged from 24 to 406. Thirty-five instruments were used in fifty studies to assess caregivers' psychological status and mood (9 instruments), burden/distress (12 instruments), quality of life (5 instruments) or other (9 instruments). Eighteen studies assessed the measurement properties of 21 instruments, most commonly hypothesis testing (known validity) and internal consistency. Study quality varied from ‘poor’ to ‘fair’ and with many properties rated as ‘indeterminate’.
Although several instruments have been used to assess the impact of COPD on caregivers, an increased understanding of their properties is needed before their widespread implementation.
