Intellectual development at age 12 years of children with congenital hypothyroidism diagnosed by neonatal screening+

Twenty-seven patients with congenital hypothyroidism diagnosed by neonatal screening were examined at the age of 12 years. The 12 patients with severe hypothyroidism at diagnosis (thyroxine <2 μg/dl (<26 nmol/L), and area-of-the-knee epiphyses <0.05 cm2 had a lower IQ than the 15 patients with less severe hypothyroidism (mean ±SD, 89±17 vs 104±10; p<0.007). Comparisons of patients and siblings confirm that this difference was due to the severity of hypothyroidism.

    Studies from the Netherlands and France29,30 have shown persistent differences in IQ where patients are followed through young adulthood; this may reflect less aggressive correction of hypothyroxinemia in neonates during the early years of newborn screening than is now practiced. Also, the poorest neurodevelopmental outcomes may be associated with the most severe forms of congenital hypothyroidism31,32 (such as very low or undetectable T4 concentrations at birth), or with lower starting doses of levothyroxine replacement that result in a longer time to normalization of serum T4 in the neonate.33 Furthermore, individual subtest components of overall cognitive function may show deficits when compared with a control population; these may include domains of attention, memory, arithmetic, and verbal skills.34-36

    Thyroid deficiencies in the antenatal and neonatal periods are often associated with delays in motor and mental function, as well as hearing defects later in life. In comparison, congenital hyperthyroidism is associated with high morbidity and mortality due to the incidence of high-output cardiac failure, hydrops, growth restriction, advanced bone maturation, and mental retardation.4–9 Fetal goitre is diagnosed by ultrasound during the second or third trimester.
