Abstract

The important role of the omega-3 fatty acids in the pathophysiology and treatment of bipolar disorder is now supported by a substantial body of indirect and direct evidence. This paper will describe the clinical and pharmacological features of bipolar disorder, review the available data regarding omega-3 fatty acids in bipolar disorder and provide recommendations for future research.

Role of functional foods and nutraceuticals for depression, post-traumatic stress disorder, and suicidal behaviors

Depression, post-traumatic stress disorder (PTSD), and suicidal behaviors are prevalent mental health disorders that can significantly affect a person's quality of life. While traditional treatments such as pharmacotherapy and psychotherapy have been effective for some patients, they may not be suitable for everyone. As such, there is growing interest in using functional foods and nutraceuticals as alternative or complementary therapies for these mental health disorders. Functional foods provide additional health benefits beyond essential nutrition, such as improving cognitive function, reducing inflammation, or regulating mood. Nutraceuticals are bioactive compounds derived from food that can have medicinal properties and are often taken in supplement form. Several functional foods and nutraceuticals have been studied for their potential therapeutic effects on depression, PTSD, and suicidal behaviors. For example, omega-3 fatty acids (O3FAs), found in fatty fish and some nuts and seeds, have been shown to have anti-inflammatory effects and may help regulate mood. Curcumin, a compound found in turmeric, has been shown to have antidepressant effects by modulating neurotransmitter levels in the brain. Probiotics, which are live microorganisms that can confer health benefits, have been shown to improve symptoms of depression and anxiety in some patients. While the use of functional foods and nutraceuticals for mental health disorders is promising, more research is needed to understand their efficacy and potential side effects fully. It is important to note that these products are not a replacement for traditional treatments and should be used in conjunction with pharmacotherapy and psychotherapy under the supervision of a healthcare professional.
In conclusion, functional foods and nutraceuticals have shown promise as alternative or complementary therapies for depression, PTSD, and suicidal behaviors. These products can provide additional health benefits beyond essential nutrition and have been shown to have therapeutic effects on mental health disorders. However, further research is needed to understand their efficacy and potential side effects thoroughly, and they should be used in conjunction with traditional treatments under the supervision of a healthcare professional.
Effects of Fish-Oil Consumption on Psychological Function Outcomes in Psychosis: A Systematic Review and Dose–Response Meta-Analysis of Randomized Controlled Trials

Research on the effects of fish oil on clinical symptoms and psychosocial functioning in people with psychosis has been inconsistent. We conducted this systematic review and meta-analysis to summarize the available data on the effects of oral intake of fish oil on psychological functioning in patients with psychosis. Three online databases including PubMed, Scopus, and Web of Science were searched to identify relevant studies published by April 2021. The exposure was oral fish-oil supplementation. The Positive and Negative Syndrome Scale (PANSS), the Brief Psychiatric Rating Scale (BPRS), and the Global Assessment of Functioning (GAF) were our outcome measures. Seventeen randomized clinical trials involving 1390 patients were included. No change in PANSS was observed following oral fish-oil intake [weighted mean difference (WMD): −0.87; 95% CI: −16.99, 15.26; P = 0.92]. In a nonlinear dose–response analysis, a significant inverse association was observed between <10 wk of fish-oil supplementation and PANSS (WMD: −10; P-nonlinearity = 0.02). Although analysis of 4 studies showed a nonsignificant reduction in BPRS after fish-oil intake (WMD: −2.990; 95% CI: −6.42, 0.44; P = 0.08), a nonlinear dose–response analysis revealed significant inverse associations between dose (>2200 mg/d) and duration of fish-oil supplementation (<15 wk) with BPRS score (WMD: −8; P-nonlinearity = 0.04). Combined effect sizes from 6 randomized clinical trials showed significant increases in GAF after oral administration of fish oil (WMD: 6.66; 95% CI: 3.39, 9.93; P < 0.001). In conclusion, we did not find any significant changes in PANSS and BPRS scores following fish-oil supplementation. Nevertheless, oral fish-oil intake significantly contributed to improvement in GAF scores. This is the first meta-analysis to examine the effects of fish oil on the psychological functioning scores of PANSS, BPRS, and GAF simultaneously.
The hypothalamic-pituitary-adrenal axis in bipolar disorder

The hypothalamic-pituitary-adrenal (HPA) axis is one of the most important biological mediators of the bidirectional communication between body and mind. Indeed, psychiatric disturbances are profoundly influenced by the activity of this neuroendocrine system and vice-versa. With the present chapter, we specifically aim to provide insights into the relationship between the HPA axis and bipolar disorder (BD). After a general overview on what the HPA axis is, we will explore the evidence up to date on its potential connection with BD. We will finally discuss the clinical relevance of such relationship, mainly focusing on therapeutic opportunities.

Bipolar spectrum disorders (bipolar I disorder, bipolar II disorder, cyclothymic disorder, and other specified bipolar and related disorder) are impairing disorders characterized by periods (episodes) of euphoria and/or intense irritability and periods of depression and/or irritability. Bipolar spectrum disorders affect both children and adults cross-culturally and are associated with impaired cognitive, social, and occupational functioning. This article briefly describes the prevalence, presentation, assessment, and treatment of bipolar spectrum disorders in children and adults.
The Effects of Omega-3 Polyunsaturated Fatty Acids on Maternal and Child Mental Health

The burden of mental health disorders in women, children, and adolescents has become a public health problem in the U.S. (Kessler, 2003). Essential omega-3 fatty acids, particularly docosahexaenoic acid (DHA, 22:6, n-3), play critical roles in building and maintaining a healthy central nervous system, affecting cognition and mood regulation. Given these roles, omega-3 fatty acids are being investigated as a complementary or alternative approach in the treatment of mental health disorders including postpartum depression, attention-deficit hyperactivity disorder (ADHD), childhood depression, and autistic spectrum disorder. The present chapter reflects the current state of scientific knowledge regarding the link between omega-3 fatty acids and general health and related psychopathologies affecting maternal, infant, and child health. Efforts are necessary to expand our current knowledge of the impact of omega-3 fatty acids on mental health disorders. Given the poor general intake of omega-3 in the general population of women and children, increased efforts are necessary to ensure intake is improved.

The relationship between essential fatty acids (EFAs) and Major Depressive Disorder (MDD), post-natal depression (PND), Bipolar Disorder (BD), suicidality and some personality variables is considered. There is evidence that omega-3 and omega-6 fatty acids (FAs) may be involved in the aetiology of some psychological disturbances, including disorders of affect and certain maladaptive behaviours such as impulsivity, aggression and suicidality. The chapter examines the role of fatty acids in affective disorders and maladaptive behaviours by describing and evaluating relevant epidemiological, correlational/ clinical and intervention studies. Finally, implications for practice and directions for future research are suggested.
