Zusammenfassung

Studienziel: Ziel dieser Studie war es, klinisch-radiologische Ergebnisse proximaler Humerusfrakturen, die mit der T-Platte osteosynthetisch stabilisiert wurden, zu erfassen. Methode: Es wurden alle Patienten mit einer proximalen Humerusfraktur eingeschlossen, bei denen die Operationsindikation gestellt wurde und die Versorgung mit der T-Platte erfolgte. Erfasst wurden subjektive Beschwerden, Komplikationen, das klinische Ergebnis mithilfe des Constant-Murley-Scores, des UCLA-Scores und des DASH-Scores sowie die radiologischen Befunde. Zwischen 12/96 und 10/03 wurden 74 Patienten (76 Frakturen) mit der T-Platte versorgt. An revisionsbedürftigen Komplikationen traten 2 Hämatome und 3 Osteosyntheseversagen unter funktioneller Nachbehandlung auf, wobei zeitnah die erneute interne Stabilisierung vorgenommen wurde. Bei 19 Patienten war wegen implantatbedingter Beschwerden eine Metallentfernung erforderlich. Ergebnisse: Nach 67 Monaten wurden 52 Patienten (54 Frakturen) nachuntersucht. Gemäß der Neer-Klassifikation handelte es sich um 34 2-Teile-, 15 3-Teile- und 5 4-Teile-Frakturen. 80 % der Patienten waren subjektiv zufrieden. Unter Berücksichtigung der Schulterscores wiesen 74 - 89 % der Patienten ein sehr gutes bis befriedigendes Ergebnis auf. Schlussfolgerung: Die konventionelle T-Platte ermöglicht die Versorgung auch komplexer proximaler Humerusfrakturen und gestattet selbst beim alten Patienten eine frühfunktionelle Nachbehandlung. Sofern die Faktoren weichteilschonende, anatomische Reposition und stabile Fixation der Fraktur mit anschließender frühfunktioneller Nachbehandlung berücksichtigt werden, kann auch bei komplexen 3- und 4-Teile-Frakturen ein gutes funktionelles Endergebnis erwartet werden.
