Zusammenfassung

Studienziel: Eine Metaanalyse der randomisiert kontrollierten Studien über den Vergleich minimalinvasiver Hüfttotalendoprothetik gegen Standardtechnik. Methode: Online-Datenbanken (Medline, EMBASE, CINAHL, CCTR) und Kongressbände wurden nach prospektiven, randomisierten, kontrollierten Studien durchsucht, die minimalinvasive Hüftendoprothetik mit dem Standardprozedere vergleichen. Daten bezüglich Operationsdauer und Blutverlust sowie Komplikationsrate und Harris Hip Score wurden extrahiert und gesammelt. Ein random effects model nach der DerSimonian- und Laired-Methode wurde angewandt um die Einzeldaten zu fusionieren. Ergebnisse: In 8 Studien wurden 481 minimalinvasive Hüften mit 436 Standardoperationen verglichen. Bezogen auf die Operationsdauer besteht kein Unterschied (durchschnittlicher Unterschied (d. U.) 4 min, p = 0,21), dafür ist der Blutverlust signifikant geringer (d. U. 43 ml, p < 0,001). In der Besserung des Harris Hip Score ist ein trendweiser Vorsprung in der minimalinvasiven Gruppe mit 4,14 Punkten zu sehen (p = 0,06), während die Odds Ratio der postoperativen Komplikationen wiederum keinen Unterschied zeigt (p = 0,71). Schlussfolgerung: Zusammengefasst zeigen sich die Unterschiede zwischen beiden Techniken nur marginal. Die minimalinvasive Hüftendoprothetik stellt sich somit als kosmetisch attraktive Variante der Standardtechnik dar. Diese Studie kann keinen Aufschluss über Unterschiede in der Rehabilitation geben.
