Zusammenfassung

Das Asthma bronchiale ist eine chronische körperliche Erkrankung, bei der psychische Faktoren eine bedeutende Rolle spielen können. Obwohl es keine Hinweise auf eine für Asthma-Patienten spezifische Persönlichkeitsstruktur gibt, konnte vor allem für die folgenden psychosomatischen Faktoren ein Einfluss auf den Krankheitsverlauf nachgewiesen werden: Schwere Anfälle können durch starke Emotionen ausgelöst werden, typischerweise im Zusammenwirken mit allergischen und anderen Faktoren. Eine depressive Komorbidität ist von großer Bedeutung, da sie die Lebensqualität und auch die Therapie-Compliance negativ beeinflusst. Non-Compliance gegenüber der antiasthmatischen Therapie und die Vermeidung oder Verzögerung ärztlicher Hilfe in Notfallsituationen sind typische Muster maladaptiven Krankheitsverhaltens, die vor dem Hintergrund der anhaltenden Mortalität im Gefolge schwerer Anfälle besondere Aufmerksamkeit verdienen. Unter den Patienten mit maladaptivem Krankheitsverhalten finden sich solche, die das medizinische Versorgungssystem überbeanspruchen, und solche, die es zu wenig nutzen. Negative Einstellungen gegenüber der antiasthmatischen Medikation, besonders einer Kortison-Medikation, und die Neigung zur Vermeidung oder Verzögerung der Inanspruchnahme ärztlicher Hilfe können Ausdruck einer psychischen Konfliktproblematik sein. Es wird empfohlen, bei allen Asthma-Patienten routinemäßig ein Interview zu führen, das es gestattet, emotionale Belastungsfaktoren, eine depressive Komorbidität und maladaptive Muster des Krankheitsverhaltens zu identifizieren. Wenn erforderlich, sollten psychotherapeutische Interventionen entsprechend der spezifischen Problemlage der Patienten eingeleitet werden.

Abstract

Bronchial asthma is a chronic medical condition where psychological factors can play an important role. While there is no evidence of a specific asthma personality, the following psychological factors have been found to influence the course of the illness: First, severe attacks can be triggered by heavy emotions, typically in conjunction with allergic or other factors. Second, depressive comorbidity is a common phenomenon which negatively affects life quality and treatment compliance. Third, non-compliance with antiasthmatic medication and the tendency to avoid or delay medical help in emergency situations are typical patterns of maladaptive illness behavior which deserve attention in light of persistent mortality from severe attacks. With regard to the utilization of medical services, patients with maladaptive illness behavior can be divided into overusers and underusers. Negative attitudes toward antiasthmatic medication, esp. cortisone medication, and the tendency to avoid medical help often reflect major psychological problems. It is strongly recommended to administer a routine interview to all asthma patients which allows for identifying emotional stressors, depressive comorbidity and maladaptive patterns of illness behavior. If necessary, psychotherapeutic interventions should be planned according to the specific needs of the patients.
